The blocklist syncs through your iCloud account. We cannot see it. No account or tracking is required.
Enable it in Safari
In Safari, choose Settings, then Extensions and enable Hold That Tab Blocker for each Safari profile you use. Help in Hold That Tab and the setup guide have the full steps.
